When you apply for a scholarship, it is important to describe what you did, list those you talked to, and get us on the same page!

When you apply, please do so in writing. We need ten days to process a completed application. Include the following information as minimum, and mail to:

  • Your name, mailing address, e-mail and phone number
  • Seminar(s) you are interested in, with dates and location, and why
  • Steps you've taken to earn the scholarship (include supporting documents, including news clippings, video and other media)
  • Steps you've taken to generate value and satisfaction in the seminar for yourself and others
  • Steps you will take to generate value and satisfaction in the seminar for yourself and others
  • Things you will do to cause excitement in the seminar
  • Things you will do to be a contribution for yourself, and for others
  • Briefly describe your passion about radiology and patient care
  • Do you agree to send a written report about the seminar within two days of the seminar, including recommendations on how to enhance the experience, for future participants?
